About the Role
Lumiere Foundation is seeking an experienced NDIS Support Coordinator to work withICCareparticipantsin greater Sydney.
In this role, you will provide varying levels of Support Coordination and Transition of Supports, assistingparticipants to implement their NDIS plans, connect with services, and achieve their goals while exercising choice and control.
In This Role, You Will
- Apply and adhere to NDIS and Child Safe standards and regulations
- Efficiently coordinate supports for participants withinallocatedfunding levels
- Liaise and collaborate with participants, families, service providers, and other stakeholders to achieve participant goals
- Develop andmaintaineffective service linkages and community connections
- Empower participants to exercise choice and control through formal, informal, mainstream, and community-based supports
- Identifyopportunities for innovative and person-centred service delivery
- Maintainaccuraterecords, case notes, files, and reports in line with NDIS requirements
- Ensure compliance with NDIS Quality and Safeguards standards and legislative requirements
WhatYou’llBring
- Tertiary qualification in Social Sciences, Health, Community Services, or a related field, or a Diploma in Disability Services
- Minimum 2 years' experience in NDIS Support Coordination
- Strong understanding of the NDIS Quality and Safeguards Commission Practice Standards
- Experience working within the disability and community services sectors
- Experience using client management systems such as MYP,Lumary,ShiftCare , or similar
- Excellent verbal and written communication skills
- Strong report writing and documentation skills
- Ability to speak Arabic is highly regarded but not essential
- Full Australian working rights
- Current National Police Check
- Current First Aid Certificate
- NSW WorkingWithChildren Check (WWCC)
- NDIS Worker Screening Check
- Unrestricted NSW Driver Licence
- Access to a reliable, insured, and roadworthy vehicle
