Uniting Communities is seeking an experienced health skilled to join our Aged Care Assessment Service as a Triage and Clinical Assessor. You will perform phone-based triage, assess clients’ medical and functional needs, and determine eligibility for care funding under the Aged Care Act.

You will work with the ACAS team to identify the best assessment pathways and support plans, using MAC and IAT tools, while documenting outcomes in the My Aged Care portal and coordinating with staff and
