Clinical Care Coordinator – Support at Home

Amplar Health (a Medibank brand) is seeking a Registered or Enrolled Nurse to join our Support at Home ( ) Clinical Pathways team. The role blends clinical expertise with care coordination to deliver safe, timely, and client‑centred services for older Australians.

What you’ll do:

- Screen and assess referrals from aged care providers.
- Allocate clients to the right clinical pathway and coordinate service delivery.
- Monitor progress, review outcomes, and ensure compliance with the Aged Care Quality Standards.
- Contribute to process design and continuous improvement as the program evolves.
- Act as a trusted clinical liaison for providers, clients, and families.

What we’re looking for:

- Current AHPRA registration (RN or EN).
- Minimum 3 years’ experience in aged care, community care, or rehabilitation.
- Strong clinical assessment and care coordination skills.
- Digital proficiency and ability to work virtually.
- Empathetic, proactive, and client‑focused approach.
