Medibank Amplar Health seeks a Cardiac Rehab Program Clinician to join the national Heart Health at Home program. You will deliver virtual cardiac rehabilitation coaching and education, manage caseloads, and develop personalised care plans for participants recently discharged after a cardiac event.
You will work autonomously in a fully remote role (0.6 FTE) with a private workspace, equipment provided, and a 12-month fixed-term contract.
