About the Role

HYPAR are looking for exceptional support workers/mentors to assist our participants to achieve their goals through leading and mentoring, and building relationships based on HYPAR’s core values.

Everyday can be a little different, but here at HYPAR the role of a support worker/mentor includes:

Implementing care plans, safety plans and behaviour support plans.

Aligning supports with allied health professional’s recommendations to assist with achieving goals and using best practice strategies.

Planning, assisting and engaging participants in suitable activities.

Support participants to engage community and mainstream service.

Provide domestic support, including but not limited to; house cleaning, laundry, cooking, budgeting, shopping, etc.

Provide mental health support within a therapeutic framework to deliver quality care.

Complete high-quality documentation to provide the NDIS and governing bodies with information relating to supports, incidences and funding needs.

Positions are available throughout the Moreton Bay region.

About You

The ideal candidate will have:

Have previous experience working with people with complex disabilities and behaviours.

Have experience working with people with psychosocial disabilities

Exceptional emotional intelligence, demonstrating initiative and flexibility, being able to build strong relationships with participants, families and stakeholders.

The ability to maintain professionalism in high-pressure situations.

Excellent communication skills – both written and verbal,



and are proficient in the use of smart phones and computers.

Robust professional boundaries, and the understanding of their importance.

A drive to do purposeful work that improves the quality of life and goal outcomes for out participants.

Ability to work a versatile roster, with availability in the mornings, evenings, and weekends. Preference to have availability to do 1-2 sleep over shifts per week. We are also looking for a mentor for overnight awake shifts.

Knowledge of NDIS Guidelines for reasonable and necessary supports, and the NDIS Commission Quality and Safeguarding Framework.

You will also be required to have or be willing to obtain:

Minimum Certificate IV or higher in a relevant qualification - Highly regarded

Disability Support Work or industry relevant experience - Highly Regarded

Paid Blue Card

Paid NDIS Disability Worker Screening (or valid yellow card)

Child Safety background screening check (LCS2)

Have access to a registered vehicle and full comprehensive insurance

Open QLD Drivers Licence (International and P-Plate Licences will not be accepted)

A working android or apple smartphone
