Central Coast Local Health District (CCLHD) has an opening for a permanent full time Registered Nurse to join the Hospital in the Home (HITH) team at Gosford, Wyong Hospitals and the Long Jetty Health Care facility. You will deliver acute clinical care to patients in their homes or HITH clinics, collaborating with a multidisciplinary team to monitor health and promote wellness.

You will educate patients and families about conditions and self-care, adapt evidence-based practice to home settings,
