About the Opportunity Join the Singleton Community Health Nursing Team and make a meaningful difference by delivering person-centred healthcare to individuals and families across Singleton and surrounding rural communities. Our team provides a broad range of community-based nursing services, including immunisations, chronic disease management, health assessments, wound care, care coordination and health promotion. Working across clinic and home-based settings, you'll support people to maintain their health, independence and wellbeing while helping improve access to quality healthcare for rural communities.

As a Community Health Registered Nurse, you'll work collaboratively with general practitioners, allied health professionals, hospitals, aged care providers and community agencies to deliver coordinated, holistic care tailored to each person's needs. What You Will

Be Doing As a Registered Nurse within the Singleton Community Health Nursing Team, you will provide comprehensive nursing care across a variety of community health settings, delivering high-quality care that supports people to remain healthy and independent within their own homes and communities. In this role, you will: Deliver community nursing services across clinic and home-based settings.

Provide immunisation services in accordance with NSW Health guidelines. Undertake comprehensive health assessments and develop person-centred care plans. Deliver chronic disease management and preventative health programs.





Perform wound assessment and management using evidence-based practices. Coordinate care and facilitate referrals to appropriate health and community services. Provide health promotion, education and self-management support to individuals and families.

Work both autonomously and collaboratively within a multidisciplinary team to achieve positive health outcomes. Maintain accurate clinical documentation and contribute to continuous quality improvement initiatives. Report directly to the Nurse Unit Manager, Singleton Community Health.

About you Our ideal candidate is a compassionate and motivated Registered Nurse with experience in community and primary healthcare who is passionate about improving health outcomes for rural communities. You enjoy working autonomously while remaining connected to a multidisciplinary team and are confident managing a varied caseload across clinic and community settings.

You will demonstrate excellent clinical judgement, solid organisational and communication skills, and a commitment to delivering evidence-based, person-centred care. Experience in immunisation, chronic disease management, wound care and health promotion will enable you to thrive in this rewarding role. Essential requirements: Current registration as a Registered Nurse with AHPRA.

Current unrestricted NSW Driver Licence. Accredited Nurse Immuniser (NSW), or willingness to complete an approved immunisation course within the first 12 months of employment. Demonstrated experience in community or primary healthcare nursing.

Ability to work independently and effectively manage a diverse caseload. Excellent communication, assessment and clinical decision-making skills.
