NSW Health is seeking a Senior Social Worker (Level 3) in the Community Palliative Care Team at Prince of Wales Hospital. The role involves providing psychosocial support to patients and carers, undertaking clinical caseload management and care planning within a multidisciplinary context.

The successful applicant will join a large, skilled Social Work Department and contribute to crisis intervention, bereavement support, and coordination with community services.
