About the Role
As a Care Partner - Support at Home, you will be responsible for care management activities for older people receiving services under government and private funding.
Each participant is allocated a dedicated Care Partner who works collaboratively with them to plan, manage, and coordinate services aligned to their aged care assessments, funding, and individual goals.
Where appropriate, and with participant consent, you will involve informal carers, supporters, and family members in the care planning process.
Participants with complex needs may have their care overseen in collaboration with a clinical care partner.
This role also contributes to intake processes, home visits, and timely onboarding of new Alpha Support at Home clients across your allocated region.
Key Responsibilities
Care Management & Service Coordination
Deliver person-centred care planning aligned to participant goals, preferences, and assessed needs
Support participants to understand their approved services and funding, enabling informed decision-making
Ensure care plans promote reablement, independence, wellbeing, and dignity of risk
Monitor, review, and adjust services and budgets in response to changing participant needs
Maintain regular participant contact, including a minimum of one care management activity per participant per month




Conduct home visits for care planning and review purposes as required
Refer participants to My Aged Care for reassessments and support plan reviews when appropriate
Ensure all care management activities align with Standard 3 of the Aged Care Quality Standards
Intake & Service Coordination
Coordinate intake and onboarding of recent clients in a timely manner
Monitor the client waitlist and manage admissions efficiently
Liaise with multiple service providers to ensure timely, high-quality service delivery
Coordinate and manage participant care planning against budgets within approved funding levels
Identify and elevate client and financial risks to the clinical and finance teams respectively
Work closely with internal Program Support, Finance, and Administration teams to resolve participant enquiries
About You
Experience working in health, aged care, or community-based settings
AHPRA Registered Nurse with aged and/or community-related field
Strong understanding of person-centred care and aged care funding models
Excellent communication, coordination, and stakeholder engagement skills
Ability to manage competing priorities while maintaining compliance and quality outcomes
