This temporary part time role (32 hours per fortnight until 4 July ****) supports screening and prioritising referrals, coordinating care, and connecting consumers with appropriate services across Barwon Health.You will work with multidisciplinary teams, uphold patient-centered care, and contribute to quality improvement.
A healthcare degree and current AHPRA registration are preferred, with strong communication
