Eastern Health is seeking a Grade 2 Social Worker to join the Early Stroke Discharge (ESD) team. You will help patients transition from bed-based rehabilitation to home, within a supportive multidisciplinary workplace.
The ESD service operates six days a week across Peter James Centre and Angliss Community Rehabilitation Centre, with care delivered in homes or centres. Part time roles may be considered.
