The team works collaboratively with consumers, families and carers, and key service partners to ensure coordinated, timely and person-centred support.As a Mental Health Clinician, you will provide recovery-oriented, community-based mental health care to individuals experiencing moderate to severe mental illness.Working within a multidisciplinary team, you will deliver assessment, treatment and care coordination, supporting consumers and their families/carers through a person-centred and collaborative approach.
The role contributes to safe, timely and coordinated care aligned with the Victorian Mental Health and Wellbeing Act ****.Key ResponsibilitiesDeliver assessment, treatment and recovery-oriented interventionsProvide coordinated, consumer and family-inclusive careContribute to intake, risk assessment and case management functionsWhat We Are Looking ForQualification in Nursing, Social Work,
