(Working from home and our office in Sippy Downs)You will be responsible for the planning and oversight of the scheduling and delivery of timely care and supports that meet customer goals and delivers exceptional service.You will work as part of a dynamic,
passionate and fun team in a fast‐growing business that offers plenty of growth and career potential.Key responsibilitiesPlanning and oversight of the scheduling of support workers and third‐party suppliers to deliver quality care and servicesWorking collaboratively with the Service Manager to undertake and ensure timely recruitment and effective performance management of staffResponsible for the development and review of Care Plans in consultation with customers and their familiesEnsure individual customer's budgets are accurately recorded and monitored regularly to reflect the customer's care planRegularly reviewing customer Care Plans, liaising with internal teams such as nurses, rostering staff and Care ProfessionalsNetwork and develop new business within the community, attend aged care events and be a leader in the communityKey selection criteria12 months experience ideally in community home care as a Care or Case ManagerHighly professional and able to act with compassionWarm sales and customer service experience, developing businessExcellent administrative and computer skillsExcellent communication skillsRequirementsSocial, Home & Community Services, Aged Care, Disability, Health Industry or related relevant experienceNursing qualification is preferred but not essentialCurrent driver's licence and secure driving recordOwn car & willingness to travel to visit customers (mileage payment provided)What we offer / Perks & BenefitsDay off for your birthdayThis is a unique opportunity to make a difference in people's lives.
