The service also delivers care to Post Acute Care and Chemotherapy patients under our Cabrini at Home program.The RoleWe are seeking an experienced Hospital in the Home (HITH) Nurse to join our team on a full-time or part-time basis, with flexible FTE options available for the right candidate.Working across a rotating 7-day roster, including some PM and on-call shifts, you will deliver high-quality acute care to patients in their homes.
This senior clinical role offers the opportunity to work both independently and as part of a multidisciplinary team, partnering with physicians, specialists and allied health professionals to provide safe, evidence-based, patient-centred care.Key ResponsibilitiesDevelop and implement comprehensive care plans tailored to each patient's needsMonitor patient progress and ensure continuity of careConduct clinical assessments, administer treatments,
