NSW Health is seeking a Care Coordinator in Palliative Care for the Coastal Network. The role provides clinical leadership and coordinates care from referral to discharge in Bega and Eurobodalla. It requires AHPRA registration as a RN with post-registration palliative care qualifications and at least three years in the field.

The position emphasizes collaboration with GPs, community nursing teams and other providers, plus mentoring staff and advancing practice within a multidisciplinary team.
