two Care Partner - Relievers
to join our Community Services team, with one position based at our
Marion
and one position based at
Noarlunga.
As a Care Partner – Reliever, you will work across our
Support at Home
program, providing additional support across care portfolios to ensure continuity of high-quality, person-centred care.
You will oversee and monitor services, review outcomes against individual care plans, and partner with older people, their families and the broader care team to support clients to remain living safely and independently in their own homes.
Essential Criteria
Experience in community aged care, care coordination, case management or a similar client-focused role.
Program Coordination, Team Leader, case management experience




An empathy and understanding of issues for older people, their families, and carers
Knowledge and experience of quality systems for evidence-based care coupled with effective written and verbal communication skills
A commitment to supporting older people to remain at home in the community
Demonstrated ability to work in a holistic, person centred model of care
An ability to work with individuals' to identify their needs, develop personal goals and develop responsive packages of services
Demonstrated computer literacy and familliarity with MS Office Suite and electronic client care systems
A current SA driver's license

The following qualifications are highly recommended:
Diploma of Nursing
Certificate III in individual Support (Ageing) or Health Services Assistance
Certificate IV in Aged Care, Disability or Community Services
Diploma of Community Services (Case Management)
Diploma of Ageing Studies and services
