This is an exciting opportunity for a Registered Nurse or Allied Health professional who is passionate about person-centred care, quality outcomes and leading others.
About the RoleReporting to the Home Care Nurse Consultant,



the Clinical Care Partner leads personalised care planning and oversees the safe delivery of services for a portfolio of clients.You will work closely with clients, families, Care Partners, Team Leaders and healthcare providers to ensure care remains responsive, compliant and aligned to each client's goals.
Key ResponsibilitiesClinical Assessment & Care PlanningConduct comprehensive client assessments covering physical, cognitive and psychosocial needsDevelop personalised care plans aligned to goals, preferences and cultural considerationsReview and approve care plans developed by Care PartnersMonitor for health deterioration and coordinate timely interventions or referralsReassess and update care plans as needs or funding arrangements changeCare Coordination & LeadershipCoordinate services with GPs, specialists, allied health providers and internal teamsSupport clients to navigate My Aged Care, Home Care Packages and Support at Home programsProvide leadership, coaching and clinical guidance to Care PartnersSupport Team Leaders to monitor Home Care Support Workers and safe service deliveryManage purchases of prescribed aids and equipmentClient & Family SupportEducate clients and families on care plans,



service agreements and available supportsPromote culturally safe, inclusive and trauma-informed careAssist with future care planning and changing support needsGovernance, Quality & ComplianceMaintain accurate documentation and clinical recordsParticipate in case reviews, audits and continuous improvement initiativesManage complaints, incidents and risk escalations appropriatelySupport compliance with aged care standards and regulatory requirementsEssential RequirementsTertiary qualifications with current AHPRA registration as a Registered Nurse or relevant Allied Health discipline in AustraliaCurrent driver's licence and access to a comprehensively insured vehicle for work useStrong clinical assessment and case management capabilityExperience supporting older people living at homeStrong knowledge of ageing, dementia and behaviour support strategiesExcellent communication, stakeholder engagement and problem-solving skillsStrong organisational and time management skillsHigh level computer literacy including Microsoft OfficeDesirableMinimum 3 years' experience in a similar roleAdditional specialty qualifications relevant to community or aged careStrong relationships across medical, allied health and community networks
