Responsibilities




Demonstrated ability to build strong partnership relationships with clientProvide comprehensive case management for each clientBuild strong Care Plans that are appropriate for the individual needs of each client whilst meeting the limitations of each client's budgetRe-assess client care plans, goals, OHS assessment, and emergency plans for clientsMaintain strong client notesNimble in their ability to support clients
About You
Ability to speak Greek language is requiredProficiency in computer skillsExcellent communication and interpersonal skillsAbility to work independently and as a part of a multidisciplinary team
