Coordinating and documenting the admission, management and discharge planning of patients seen in the Unit.The day-to-day assessment and management of the patients in the Unit, communicating with them and their family the nature of their condition and its treatment.
Attend daily case conferencing or as required.
Communicating and coordinating care with their team, arranging consultation with other staff when CGA of Hip fracture patients presenting to ED.All decisions concerning the patient(s) in consultation with their immediate supervisor and team.
Participating in the virtual aged care service (VACS) with responsibility for the medical management of patients under the service in the community.
The community responsibilities will underpin the hospital avoidance strategies and will include medical assessments at home ie includes community or Residential Aged Care Facilities (RACF).
Continuity of care for the VACS patients between hospital and community to optimise patient recovery and optimise health.
Providing clinical governance to Community Staff working with the target population.
Where you'll be workingThe Nepean Aged Health Service provides a comprehensive array of services that cover the continuum of care across acute subacute,



Geriatric Outreach services, in community including residential aged care.
56 and 4 ACRAU-MAUacute beds and 16 geriatric rehabilitation beds and 6 GEM beds.
Acute services include:ACRAU - Aged Care Rapid Assessment Unit (4 beds) A multidisciplinary ASET team Two Acute Aged Care WardsOrthogeriatric service COPS - Care of Older Person in SurgerySubacute services include: Specialty clinics (Cognitive Disorders, General, Comprehensive Falls and Fractures) and PrEPP Springwood ***** subacute bedsAged Care Assessment Team VACs program provides Geriatric Outreach service in community and in RACFs via Single Point of Contact Innovative telehealth pilots with RACFs and outlying facilitiesEssential requirements MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia.
Demonstrated clinical practice as a registered medical practitioner in the last 2 years in a setting appropriate to the needs of the position.Minimum four years postgraduate experience, with at least one year experience in Aged Care within Australia.
This position is subject to an appropriate supervision level which can be supported by the health agency.
If the incumbent's registration conditions include supervision requirements,



the level of supervision must be able to be supported by the health agency for this role.Valid Working with Children Check, cleared National Police Check.
To be employed by NSW Health you are required to provide evidence of vaccination to comply with the NSW Health Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases Policy, this includes any amendments made from time to time.Selection criteria MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia.
Demonstrated clinical practice as a registered medical practitioner in the last 2 years in a setting appropriate to the needs of the position.Minimum four years postgraduate experience, with at least one year experience in Aged Care within Australia Demonstrated ability to work in a multidisciplinary clinical care team Evidence of continued professional development and commitment to self-education.Demonstrated good organisational, time management skills, excellent communication and interpersonal skills.
