This is a part-time position (22.8 hours per week), 12-month contract (ending, 19/09/2027), acting as part of the Homelessness Hub team (within the Housing and Homelessness Stream), you will support people who are homeless or at risk of homelessness.
This service is a crisis service that focuses on supporting individuals facing difficulties in accessing support, this includes supporting homeless individuals who may be sleeping rough or couch surfing, and those with complex needs.
This role aims to provide easy access to information, referrals and advocacy as well as assisting with referrals to housing and accommodation options and assistance with crisis resolution.
This role involves working with other stakeholders both internally and externally engaging the appropriate support needed per individual / couples or family.
Duties & Responsibilities
Provide center-based access and support whilst working within case management framework to provide culturally safe support to clients who are experiencing homelessness.
Conduct intake and comprehensive assessments to identify risks, strengths, and barriers to long term housing.
Work to understand client needs and work to create individualised case plans and risk assessments.
Develop and implement individualised support plans providing crisis support, address housing instability, financial management, and wellbeing goals.
Provide assistance with household budgeting, referrals, information on housing pathways, advocacy and any other supports identified during engagement.
Liaise and advocate with landlords, property managers, government departments, and community service providers to support positive outcomes.
Complete referrals to other programs and services and collaborate with a range of agencies and stakeholders both internally and externally to achieve best outcomes for clients.
Support clients to access relevant services such as mental health, AOD, financial counselling,



and family support programs.
Maintain accurate case notes, documentation, and data reporting in line with organisational and funding requirements.
Include clients in decision making, encourage feedback and provide opportunities for input.
Contribute to a positive, collaborative team culture that promotes dignity, empowerment, and client-centered practice.
To be successful you will need
Tertiary qualifications in Community / Human Services
Prior experience working with clients from diverse backgrounds and in providing support, case work and or practical assistance to people experiencing homelessness and or a crisis.
Demonstrated ability to establish rapport with people experiencing homelessness, distress, disconnection from family,domestic/family violence, alcohol or other drug challenges and or mental health issues.
Experience in risk assessment, crisis management, incident reporting and skills in navigating solutions to challenges.
Ability to coordinate / deliver activities that support development of practical, relational and or tenancy related living skills.
Ability to collaborate with homelessness, health and medical services, First Responders and other service providers.
Good communication, numeracy, administration, data entry and computer skills and the ability to learn IT systems.
Ability (and willingness) to comply with policies, procedures, established practices and contract deliverables.
Commitment to diversity, inclusion, reconciliation and closing the gap objectives and experience working with First Nationspeople,



organisations and communities.
Compliance
Evidence of relevant Tertiary Qualifications Community / Human Services
Satisfactory outcome of a current (less than 3 months old) National Criminal History Check
Blue Card (QLD working with children card)
Current QLD drivers' licence 'Class C' or interstate equivalent
Out of hours work may be required from time to time
Required to transport clients and or to visit clients in their place of residence or within another setting.
Ability to undertake local area / regional travel is required.
First Aid certificate and CPR certificate.
