About the Role: We are seeking experienced Registered Nurses to deliver comprehensive aged care assessments in Adelaide, SA.
2 positions available.
This is a hybrid role combining half telehealth and half face-to-face assessments, offering variety and flexibility.
You will conduct comprehensive assessments to determine the care and support needs of older Australians.
Working as part of a multidisciplinary team, you will undertake holistic assessments considering clients' health, functional capacity, social circumstances and overall wellbeing, helping them access appropriate aged care services and supports.
Conduct comprehensive aged care assessments across both telehealth and face-to-face settings, in accordance with relevant assessment frameworks and guidelines Assess clients' clinical, functional, cognitive, psychosocial and environmental needs Develop recommendations and support plans to facilitate access to appropriate aged care services Complete detailed assessment documentation and reports Work collaboratively with multidisciplinary clinicians and stakeholders Benefits: $85,000 – $105,000 per annum + superannuation 2 positions available Hybrid role — half telehealth, half face-to-face Full-time or part-time opportunities available (minimum 4 days per week)
