Position Profile: In
consultation with the Manager Population Health South develops,
coordinates, plans, implements and evaluates social work services
and promotes an interagency holistic approach to care of families
and individuals with complex social issues in the communities
serviced by the Population Health South.
This is a Permanent Full Time appointment at
76 hours per fortnight.

Country Health Service (WACHS) is the largest country health system

The suitability requirements below are derived from
the Job Description Form and define what will be assessed to
determine your suitability for the role:
1. Tertiary qualifications in
Social Work and eligible for full membership of the Australian
Association of Social Workers or the Society of Professional Social
Workers.
2. Demonstrated ability to apply
knowledge and experience in area of social work at an advanced
level (including crisis intervention and case work).
3. Demonstrated highly developed
interpersonal, oral and written communication skills (including
negotiation and conflict resolution).
4. Demonstrated ability to work
independently (including leadership, organisational, caseload
management and problem solving skills) and in a multidisciplinary
team
