About the Role: We are seeking experienced Physiotherapists to deliver comprehensive face-to-face aged care assessments across Perth, WA.
10 positions available across the metro area: Metro Perth - 5 positions North & East Perth - 3 positions South Perth - 2 positions In this face-to-face role, you will conduct comprehensive assessments to determine the care and support needs of older Australians.
Working as part of a multidisciplinary team, you will undertake holistic assessments considering clients' health, functional capacity, social circumstances and overall wellbeing, helping them access appropriate aged care services and supports.
Conduct comprehensive face-to-face aged care assessments in accordance with relevant assessment frameworks and guidelines Assess clients' clinical, functional, cognitive, psychosocial and environmental needs Develop recommendations and support plans to facilitate access to appropriate aged care services Complete detailed assessment documentation and reports Work collaboratively with multidisciplinary clinicians and stakeholders Benefits: $85,000 - $105,000 per annum + superannuation Multiple positions available across metro, north, east and south Perth Full time or part-time opportunities available (minimum 4 days per week)

About You: Relevant degree in Physiotherapy Current AHPRA registration Strong communication and clinical assessment skills Experience in aged care, community health or rehabilitation highly regarded Ability to work autonomously in the community Excellent documentation and stakeholder communication skills Current driver's licence and own vehicle Eligible to work in Australia
