About the role:
At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences.
Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents' conditions and escalat[??] observations and concerns.
Our dedicated Hawthorn team is looking to add passionate personal care workers to their dedicated and professional care team on a Part-Time basis across our morning and afternoon teams on a fixed fortnightly roster:
07:00am to 15:00pm:
Wednesday, Thursday, Friday and Saturday (both weeks)
Thursday, Friday & Saturday (both weeks)
14:45pm to 22:00pm:
Wednesday, Thursday, Friday and Saturday (both weeks)
Thursday, Friday & Saturday (both weeks)
In this role, you will:
Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
Provide personal care to residents with complex needs such palliative care, dementia support, restorative care and pain management to enhance their wellbeing
Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
Monitor the cleanliness, safety, environment and promptly report issues of concern
About you:
This is the place for you if you have:
Certificate III in Individual Support, or equivalent (Aged Care)
Experience in residential aged care/ home care (desired)
Experience with dementia care and palliative care support (desired)
Demonstrated abilities to support residents in a rapid paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners
Are committed to being part of a workplace culture based on theBESTway of life (Believe, Excel, Support and Trust)
Essentials for working in Aged Care:
A current Working With Children Check card
Current and ongoing flu vaccinations
Ability to pass a national police clearance and NDIS Clearance
