Kinder Caring is seeking an experiencedClinical Case Manager (Registered Nurse) to join our team on a part-time basis, covering clients across Aged Care, Disability and injury programs in the Melbourne region.
You will provide clinical nursing procedures, manage client intake and assessments, develop care plans, deliver case management, and coordinate service delivery while training care workers.
A mix of work from home, home visits and office-based tasks is expected.
