Coast & Country Primary Care is seeking a Home and Community Support Worker to assist participants with disability in daily living, personal care, and community participation.
You will work with participants, families and professionals to implement NDIS plans, support goal achievement, and promote independence.
Minimum 20 hours per week with flexible hours, a driver's license, and smartphone rostering competency are significant.
