Calvary Care in Sydney is advertising for a full-time Case Manager to join our team.
Our office is based in Kogarah but we have clients all throughout NSW.
Case Management experience is preferred and Diploma of Community Services or Cert IV is essential.
Your main duties will include assessing client needs, developing a cooperative, strengths-based Care Plan with the client and their carer/representative and implementing services which will meet their individual needs.
