We are currently seeking aCare Partnerto join our Care Services team and support participants and clients receiving services under theSupport at Home (SaH)program and theCommonwealth Home Support Programme (CHSP).
In this role, you will work closely with participants, families, registered supporters, and health professionals to coordinate and monitor care that supports wellbeing, independence, and reablement.
You will manage a portfolio of participants, ensuring services are delivered in line with assessed needs, approved funding, and Consumer Directed Care (CDC) principles, while maintaining robust compliance with aged care and organisational requirements.

Key responsibilities
Build robust, respectful relationships with participants, families, and carers
Coordinate, monitor, and review care plans to support independence and wellbeing
Work collaboratively with internal teams and external health professionals
Manage participant budgets, including monitoring expenditure and supporting informed choice
Ensure timely, accurate documentation, reporting, and compliance with program requirements
Respond proactively to changes in participant needs and elevate concerns as required
Contribute to a positive,
values‐led team culture focused on quality and continuous improvement
About you
You are organised, compassionate, and confident working with people in their homes and communities.
You bring a balanced approach — combining strong relationship skills with sound administration, judgement, and accountability.
You will have:
A qualification in Health, Welfare, Nursing, Social Sciences, or equivalent experience in a similar role
Experience working in aged care, community services, disability, or a related sector
An understanding of Consumer Directed Care and participant‐centred practice
Strong communication, problem‐solving, and time management skills
Confidence managing care documentation and budgets
A current NSW Driver's Licence
NDIS Worker Screening Check and Working With Children Check
