You will coordinate care planning, advocate for client choice, and ensure services meet funding and client needs, while working with a supportive team.
The role requires a Diploma or Certificate in relevant fields, Australian driver's licence, and a First Aid certificate.
You will travel between sites and communities, contributing to protected, high-quality care.
