About the Role
An exciting opportunity exists for an experiencedGrade 2 Dietitianto join St Vincent's Hospital Melbourne'sHealth Independence Programs (HIP)team.
Working within a supportive multidisciplinary environment, you'll deliver high-quality, person-centred, goal-oriented dietetic care to clients across Community Rehabilitation and Specialist Clinics.
You'll play an important role in supporting people recovering from illness or injury, living with chronic conditions, or requiring specialist outpatient services to achieve their health goals and maximise independence.
This position may be required to work across any of the HIP hub sites or programs, based on service requirements and your skills and experience.
Salary:AJ1 - AJ4 ($94,****** - $106,****** per annum, pro rata for part time).
Your Contribution
To be successful in this role, you will have:
Qualifications that provide eligibility for full membership of Dietitians Australia and participation in the Accredited Practising Dietitian (APD) Program
A minimum offour years' post-graduate experiencein a relevant clinical dietetics setting
Demonstrated advanced clinical assessment and management skills
An understanding of the Health Independence Program model of care, including person-centred, goal-directed and time-limited episodes of care
Highly developed organisational, planning and problem-solving skills
Excellent communication, collaboration and interpersonal skills, with the ability to work effectively within multidisciplinary teams
A current Victorian Driver Licence

About Health Independence Programs
This position sits withinHealth Independence Programs (HIP), a multidisciplinary service that delivers integrated, coordinated care across the hospital and community interface.
HIP aims to improve client outcomes, reduce duplication of services and support timely hospital discharge through person-centred care planning.
St Vincent's Health Independence Programs operate across several locations, includingFitzroy, Kew and Fairfield, in partnership withNorth Richmond Community Health (NRCH).
Community Rehabilitation services support people recovering from illness or injury, those living with chronic disease, and people experiencing frailty to regain function, maximise independence and improve quality of life.
This role also provides dietetic services to clients attending specialist outpatient clinics, including:
Continence Clinic
Falls Clinic
Osteoarthritis Clinic
