About the Role
My Heart Cardiology () is seeking an experienced
Nurse Practitioner (NP) or Registered Nurse (RN)
to join our team, based principally at our Richmond practice with
weekly outreach visits to partner GP clinics in the Macedon Ranges and Melbourne's northern/western suburbs
to help deliver our
cardiovascular disease Prevention Quality Improvement Program
locally. Alongside this program work, you'll take on the broader clinical duties of a nurse in a busy cardiology practice — this is a varied, hands-on role, not a single-focus position.
Location & Travel
Principal place of work:
Richmond, VIC
Weekly travel to GP clinics in the Macedon Ranges and other locations in Melbourne's Northern / Western suburbs
to support on-ground delivery of the program (a current driver's licence and willingness to travel regionally is required)
About the Program
This program is designed to improve identification and management of patients with atherosclerotic cardiovascular disease (ASCVD), with a focus on optimising LDL-C management and secondary prevention pathways. You will work closely with our cardiology team to drive measurable improvements in patient outcomes and care processes.
Key Responsibilities
Support patient identification and risk stratification for secondary ASCVD prevention
Assist with LDL-C management and therapy optimisation activities
Contribute to quarterly reporting on program progress
Participate in the virtual lipids management clinic
Participate in quarterly review meetings covering clinical outcomes and operational learnings
Support development of a final outcomes report, including recommendations for program sustainability
Assist cardiologists in delivering educational sessions to local GP practices on ASCVD and lipid management
Plus, general cardiology nursing duties, including:
Pre- and post-procedure patient preparation and care (e.g. angiography, device implants, stress testing)
Chaperoning and assisting cardiologists during consultations and procedures when needed
ECG recording and basic interpretation, Holter monitor fitting, and other diagnostic support when needed
Patient education on medications, lifestyle modification, and chronic disease self-management
Triage of patient phone queries and coordination of specialist referrals
Wound care and device (e.g. pacemaker) follow-up checks where required
Maintaining clinical records and supporting practice accreditation/quality standards
About You
Current AHPRA registration as a Nurse Practitioner or Registered Nurse
Experience in cardiology, chronic disease management, or a related clinical setting preferred
Solid understanding of cardiovascular risk management and lipid‐lowering therapies (highly regarded)
Excellent communication skills, comfortable working with patients, GPs, and program stakeholders
Confident with data collection, reporting, and quality improvement processes
A collaborative, patient-centred approach to care
