About the Role
AtHealth and Community Services (HCS), we are seeking a skilled, compassionate and motivatedBehaviour Support Practitionerto join our growing multidisciplinary team.
In this role, you will work with NDIS participants who require behaviour support, using evidence-based and person-centred approaches to improve quality of life, increase independence and reduce behaviours of concern.
You will work collaboratively with participants, families, support teams and other allied health professionals to develop and implement high-qualityPositive Behaviour Support Plansin accordance with NDIS requirements.
Key Responsibilities
Conduct comprehensiveFunctional Behaviour Assessments (FBAs)to understand behaviours of concern, contributing factors and support needs.
DevelopInterim and Comprehensive Behaviour Support Plansthat are evidence-based, person-centred and consistent with NDIS requirements.
Develop practical behaviour support strategies aimed at improving participants' quality of life, independence and community participation.
Provide behaviour support interventions to participants across home, community and supported living environments.
Provide training, coaching and ongoing guidance to families, carers, support workers and other stakeholders in implementing behaviour support strategies.
Monitor participant progress and evaluate the effectiveness of behaviour support strategies.
Review and update Behaviour Support Plans based on participant outcomes, data and changing needs.
Work collaboratively withOccupational Therapists, Support Coordinators, psychologists, nurses, support workers and other allied health professionals.
Support the reduction and elimination ofrestrictive practiceswherever possible and ensure appropriate reporting and documentation where restrictive practices are used.
Maintain accurate, timely and compliant clinical documentation, progress notes and reports.
Participate in case conferences, stakeholder meetings and multidisciplinary team discussions.
Maintain a strong understanding of theNDIS Quality and Safeguards Commission requirementsrelating to behaviour support and restrictive practices.
Contribute to continuous improvement, clinical governance and best-practice service delivery within HCS.
About You
You are a passionate and clinically confident practitioner who enjoys working collaboratively with participants and their support networks.
You have strong assessment, communication and report-writing skills and are comfortable managing a diverse community-based caseload.
Essential
Relevant tertiary qualification inPsychology, Social Work, Occupational Therapy, Nursing, Behavioural Science, Counselling, Disability, Education or another relevant discipline.
Suitable experience and/or qualifications to practise as an NDIS Behaviour Support Practitioner.
Registration or suitability with theNDIS Quality and Safeguards Commission as a Behaviour Support Practitioner, where applicable.
Understanding ofPositive Behaviour Supportand functional behaviour assessment.
Strong clinical reasoning, assessment and report-writing skills.
Understanding of behaviours of concern and restrictive practices.
Excellent communication and stakeholder engagement skills.
Ability to work independently and effectively manage a diverse caseload.
Experience working within the NDIS framework is highly desirable.
Requirements
Current Driver's Licence.
Reliable vehicle and appropriate vehicle insurance.
Working with Children Check.
NDIS Worker Screening Check, or willingness to obtain one — support will be provided.
Current police check where required.
Ability to undertake community-based appointments across participants' homes and other service settings.
