Access Health and the St Kilda 24/7 Needle & Syringe Program provide essential, compassionate healthcare to people experiencing homelessness, housing insecurity and the impacts of drug use.
About the role
The Community Health Registered Nurse position ensures individuals who face barriers to mainstream healthcare receive timely, respectful, evidence‐based nursing support that improves health outcomes and strengthens pathways into ongoing care. As a Community Health Registered Nurse, you will have the opportunity to deliver comprehensive nursing care to people attending Access Health, the Access Health Van and the St Kilda 24/7 Needle & Syringe Program.
You will complete intake and assessment for new clients, support existing service users with their health needs, and work collaboratively with co‐located clinicians to provide integrated, person‐centred care. Reporting to the Harm Reduction Program Manager, this is a Permanent Full-Time role based onsite in St Kilda, VIC. Conditions Compensation isin accordance withtheNurses Award - Level 2.
How you will make an impact
Provide frontline healthcare to people accessing Access Health, the Access Health Van and the St Kilda 24/7 Needle & Syringe Program




Strengthen referral pathways between outreach programs and Access Health GP services
Deliver harm‐reduction nursing using non‐judgemental, evidence‐based approaches that keep clients at the centre of care
Complete intake and triage for new clients and provide secondary oversight to ENs
Support clinical operations including LAIB administration, psychotropic depots, phlebotomy, immunisations and referral follow‐ups
Provide holistic clinical responses including assessment, care planning, coordination, supported referral and health promotion
Engage flexibly in outreach settings, public spaces, meals programs and drop‐in environments
Contribute to continuous improvement through program evaluation, reflective practice and identifying service gaps
Build sector capability by offering secondary consultation, triage training and clinical incident response support
Maintain high‐quality documentation aligned with funding and legislative requirements
What you will bring
A tertiary qualification as a Registered Nurse (RN Div 1) with current AHPRA registration
Demonstrated understanding of homelessness, drug use and mental health
Knowledge of relevant nursing legislation and ability to apply it in practice
Experience working with vulnerable people in community settings




Immunisation certification (desirable)
Sexual health nurse certification (desirable)
A current Employee Working With Children Check, and Victorian Driver's Licence
