Community Support Workers
to support NDIS participants across Melbourne's Western and Northern suburbs. Through person-centred support, you'll help participants achieve their goals, build confidence, develop independence, and engage meaningfully in their communities.

About the Role
As a
Community Support Worker,
you will provide person-centred support that enables participants to live independently, achieve their goals, and engage with their community.
Supporting participants with daily living activities
Assisting with community access and social outings
Transporting participants to appointments, education and activities
Encouraging independence and life skills development
Providing personal care support where required
Promoting participant choice, dignity, and inclusion
Maintaining accurate documentation and progress notes

Flexible and committed to delivering high-quality support
NDIS Worker Screening Check (or willingness to obtain)
NDIS Worker Orientation Module Certificate
Current Working with Children Check -Employee
Current First Aid & CPR Certificate
Current Australian Driver Licence
Reliable vehicle with comprehensive insurance
Current National Police Check
Smartphone and basic computer skills
Certificate III or IV in Individual Support, Disability, Community Services, Mental Health, or similar
Previous experience in disability support, aged care, mental health, or community services
