Core Support Worker – Bring the Spark to Participants' Lives (Part-Time)
Meta Healthcare is a disability and allied health provider based in Western Sydney. We support NDIS participants to build independence, confidence, daily living skills, social connection, and meaningful routines in the community and at home.
Our team includes Support Workers, Team Leaders, Physiotherapists, Occupational Therapists, Exercise Physiologists, Speech Pathologists, and mentors who work together to help participants make real progress.
This is not just a "hang out" role.
We are looking for experienced, reliable Support Workers
who understand the responsibility of supporting people with disability, following plans properly, communicating clearly, and helping participants work toward real goals.

The Role
As a Core Support Worker at Meta Healthcare, you will provide practical, person-centred support to NDIS participants across daily living, community access, skill-building, personal care, transport, day programs, and goal-focused activities.
You will work directly with participants while staying connected with families, guardians, coordinators, Senior Support Workers, and allied health professionals.
The right person for this role takes initiative, plans sessions properly, writes meaningful notes, follows instructions, communicates professionally, and understands that consistency matters.
What You'll Do
Daily Living Support
You may support participants with:
Personal care, including showering, grooming, dressing, continence support, and hygiene routines
Meal preparation and mealtime support
Medication prompts or routines, where applicable
Light domestic tasks such as laundry, cleaning, tidying, and organising
Building independence through everyday tasks, not just doing everything for the participant
Community Access & Skill Building
You will help participants access the community and build confidence through activities such as:
Shopping, appointments, transport training, gym sessions, coffee outings, sports, library visits, markets, cooking, art, and social activities
Practising money handling, using public transport, reading timetables, making shopping lists, and developing communication skills
Encouraging participants to try new things safely and confidently
Supporting participants to attend and engage in Meta Healthcare Day Programs
Goal-Focused Support
This role is about helping participants make progress, not just filling time.
You will be expected to:
Understand the participant's goals and support needs
Plan sessions with purpose
Encourage independence and choice
Recognise small wins and build on them
Support therapy strategies from allied health professionals
Report concerns, risks, changes, and progress clearly
Documentation & Communication
You must be confident writing clear progress notes and giving useful handovers.
This includes:
Writing notes that explain what happened, what support was provided, how the participant engaged, and what goals were worked on
Communicating professionally with families, guardians, coordinators, Senior Support Workers, and allied health staff
Reporting incidents, risks, cancellations, changes in presentation, or concerns promptly
Keeping communication respectful, factual, and participant-focused
Mentorship & Support
You will be supported by Senior Support Workers and allied health professionals, but you must also be able to take responsibility for your own work.
Senior Support Workers may support you with:
Onboarding and shift expectations
Session planning
Problem-solving
Progress note feedback
Practical strategies for working with participants
Building consistency and confidence in the role
Our allied health team may provide guidance around:
Communication strategies
Routines and independence skills
Safe movement and exercise programs
Behaviour support approaches
Environmental setup and therapy recommendations
What Success Looks Like
Success in this role means participants are supported safely, respectfully,
and with purpose.
This may look like:
A participant completing part of their morning routine more independently
Trying a new recipe or helping prepare a meal
Communicating needs more clearly
Attending a community activity with more confidence
Building tolerance for new environments
Practising transport, shopping, money handling, or social interaction
Following therapy recommendations in a real-world setting
Families and allied health receiving clear, helpful handovers
Progress notes showing genuine support, not generic comments
Who We're Looking For
We are looking for Support Workers who are:
Experienced in disability, community support, aged care, mental health, or similar support roles
Reliable, punctual, and consistent with rosters
Mature, professional, and able to take the role seriously
Confident supporting participants with personal care and daily living tasks
Able to plan engaging, purposeful shifts
Calm under pressure and able to use de‐escalation strategies
Good communicators with participants, families, and team members
Comfortable receiving feedback and applying it
Able to work independently while still following plans and instructions
Respectful of boundaries, privacy, dignity, and participant choice
Must Haves
Previous experience in support work, disability, aged care, mental health, community services, or a similar care role
NDIS Worker Screening Check, or willingness to obtain
First Aid and CPR
Driver's licence
Reliable vehicle
COVID-19 vaccination compliance
Working With Children Check, where applicable
Ability to write clear and professional progress notes
Willingness to complete required onboarding and training
Bonus Points
Certificate III or IV in Individual Support, Disability, Community Services, Mental Health, or related field
Experience with personal care
Experience with community access
Experience supporting participants with complex needs
Experience following therapy plans or behaviour support strategies
Confidence working with families, guardians, and multidisciplinary teams
