About the role
We have an exciting opportunity for a dynamic and enthusiastic Grade 2 Social Worker to join our Early Stroke Discharge (ESD) team. Working within a supportive multidisciplinary environment, you will play a key role in helping patients transition successfully from bed-based rehabilitation services back to their own homes.
The ESD service delivers community-based multidisciplinary care six days a week through both ambulatory and bed substitution models. Services are provided across two Eastern Health sites, Peter James Centre (Burwood East) and Angliss Community Rehabilitation Centre (Upper Ferntree Gully), with care delivered in patients' homes and/or within our centres. Our team of 14 staff members provides services across the Eastern Health catchment area, fostering a cooperative and supportive workplace culture.
This position is based across both the Peter James Centre in Burwood East and the Angliss Community Rehabilitation Centre in Upper Ferntree Gully Monday to Friday. Part-time applicants are encouraged to apply, with the possibility of a job share arrangement also available.
About you
To be successful in this role you will possess:
Eligibility for membership with the Australian Association of Social Workers (AASW).
Experience and interest in neurological rehabilitation.
Strong commitment to high-quality patient centred care in direct practice and group work.
Clinical skills in managing a complex caseload using a client-centred approach.
High level communication skills, both written and oral, to enable development of a good rapport with clients and staff.
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role, and an offer is conditional on this being up to date prior to employment.
