Registered Nurse - Chronic Disease ManagementTotal Veteran Healthcare is seeking a highly skilled and compassionate Registered Nurse to join our dedicated team of healthcare professionals.

If you have a passion for helping others and the expertise to develop and manage patient care plans, we would love to hear from you.About the Role & ResponsibilitiesAs a Chronic Disease Management Registered Nurse, you will play a vital role in supporting patients with chronic and complex health conditions through comprehensive care planning, education, and ongoing clinical coordination.
Working closely with our General Practitioners and multidisciplinary healthcare team, you will help patients achieve better health outcomes while ensuring care is proactive, evidence-based, and patient focused.
Some of your key responsibilities will include:Develop, implement, and review Chronic Disease Management plans including Coordinated Veteran Care (CVC) Plan and other health assessments in accordance with Medicare guidelines.Holistic Health Management: Monitor and manage chronic conditions, coordinate allied health & specialist referrals, and ensure continuity of care across all providers, including referrals to in-house veteran support organisationsCollaborate closely with General Practitioners and the multidisciplinary team to ensure continuity of care.Maintain accurate and timely clinical documentation using Best Practice software.Advocate for Veterans: Serve as a key advocate for veterans, ensuring their voices are heard and their healthcare needs are metSupport and Educate:



Provide education and support to veterans and their families, empowering them with the knowledge to manage their health effectivelyCollaborate Across Teams: Work closely with the multidisciplinary team to ensure seamless coordination of care, enhancing the overall healthcare experience for our veteransThings that will set you up for success:Registered Nurse (RN) with current AHPRA registrationAustralian work rights are required for this positionExperience with Best Practice software (highly regarded)2-3 years+ General Practice nursing experience (highly regarded)Chronic Disease Management (highly desirable)Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary teamStrong organisational and time-management skills, with the ability to prioritise tasks and manage competing demandsA commitment to delivering gold standard patient-centred care and a passion for improving patient outcomesA true team player who thrives in team-oriented environments and is eager to contribute to a purpose-driven team committed to making a differenceAn understanding of Medicare and DVA (highly regarded)Full Administrative and Nurse supportCollaborative work environmentOpportunities for career development and career growthThe satisfaction of making a positive impact on the lives of patients and their familiesPositive Team CultureTotal Veteran Healthcare is an equal opportunity employer and is committed to fostering a diverse and inclusive workplace.
