Located in the vibrant heart of Darlinghurst, we are positioned just minutes from Kings Cross station, with direct, high-frequency bus connections from the east, and seamless links from Gadigal and Museum stations right to our doorstep.About the roleDeliver high-quality nursing care to patients in their homes and clinic settings following discharge from hospitalBe part of a team promoting recovery, independence, and safe community living for patientsHelp reduce avoidable patient hospital presentations and readmissionsAssess, plan, implement, and evaluate care using a reablement approach that supports recovery and maximises patient independenceProvide wound care, clinical monitoring, health education, and community-based support within scope of practiceWork autonomously while recognising clinical risks, escalating concerns appropriately,
