Employment Type: Permanent Full-Time (80 hours per fortnight) with a monthly ADO.

About the role
South West Healthcare is seeking a compassionate and skilled Mental Health Clinician to join our Portland Community Mental Health Service in an ongoing full-time position.
Applications are welcomed from Registered Psychiatric Nurses, Occupational Therapists, Social Workers and Psychologists.
As part of a multidisciplinary team, you will provide specialised mental health care and recovery-focused support to adults aged ***** across the Portland region.
Working alongside consumers, families, carers and community supports, you will deliver person centred, trauma informed care that promotes recovery, resilience and wellbeing.

Key Responsibilities
Conduct comprehensive mental health assessments and develop individualised recovery and treatment plans
Manage a clinical caseload and provide outreach-based support within the community
Deliver evidence based therapeutic interventions to consumers, families and carers
Collaborate with multidisciplinary colleagues to provide coordinated and holistic care
Build and maintain strong partnerships with community services and stakeholders
Contribute to service improvement initiatives and the ongoing development of mental health programs
Maintain accurate and timely clinical documentation and reporting




Participate in clinical supervision, professional development and continuous learning opportunities
What you will bring:
To be successful in this role, you will have:
A recognised qualification and current registration in one of the following disciplines:
Registered Psychiatric Nurse (AHPRA)
Occupational Therapist (AHPRA)
Social Worker (eligible for AASW membership)
Demonstrated clinical skills and knowledge in mental health practice
Strong communication and interpersonal skills, with the ability to build effective therapeutic relationships
Excellent organisational and time-management abilities
The capacity to work autonomously while contributing positively to a multidisciplinary team
A genuine commitment to recovery-oriented, trauma-informed mental health care
