Home Caring is a reputable and fast-growing provider of NDIS and Aged Care services, nationwide.
We are currently seeking experiencedCase Manager.
The Case Manager will support a growing number of clients in the Mornington Peninsula and surrounding area.
This role is a hybrid role and is a mix of WFH traveling to clients in the community across and in the office coordinating associated administration and engagement events.This role is ideal for someone who is community-minded, culturally sensitive, and committed to providing high-quality, person-centred care.

You'll be working with clients across both Aged Care and Disability sectors.Key ResponsibilitiesConducting client assessments and developing person-centred care/support plansCoordinating services, monitoring outcomes, and reviewing care plans regularlyAdvocating for clients and ensuring their cultural, linguistic, and health needs are metSupporting clients to access appropriate services and resourcesLiaising with families, support workers, and health professionalsEnsuring compliance with the NDIS & Aged Care quality standardsEnrolling and management of Home Care Packages for Aged Care clients and NDIS funding for Disability clientsWhat We're Looking ForQualification in Nursing (Registered Nurse) and/or relevant Case Management experience.Fluency ina second language such as Mandarin, Vietnamese, Persian or Arabic is highly desirable.Experience in aged care, disability, or community health settings.Robust communication, problem-solving, and organisational skills.Understanding of NDIS and/or Aged Care systems.Empathy, cultural awareness, and a passion for helping others.Current AHPRA registration (for RNs.)Current First Aid & CPR, National Police Check, and NDIS Worker ScreeningDriver's Licence and access to reliable vehicle.Full working rights in Australia and COVID-19 vaccinationWhy Join Us?
