Hawkesbury District Health Service is a teaching hospital in Windsor NSW, providing acute, community and allied health services for the local community and surrounding districts.
The service provides 24-hour emergency, medical, surgical, diagnostic, maternity, neonatal, paediatric, palliative, intensive and coronary care services The Discharge Patient Flow Concierge Officer is responsible for supporting administrative processes for discharge planning and assist patients and their families with the process of leaving hospital and transitioning back to their home or other care setting.
The position holder will do this by strengthening current discharge planning processes in collaboration with the multidisciplinary team to expedite timely discharge and facilitate integration with out of care hospital and primary care services where appropriate.
You will be responsible for facilitating and enhancing communication between the patient, families, carers, clinical staff, out of hospital care services and primary care to improve the discharge experience for patients, carers and families.
