About the Role

Reporting to the Operations Manager, you'll own the day-to-day of how care actually gets delivered. The role that keeps clients, support workers and our growing Delivery Partner network all moving in the same direction.

You'll be responsible for:

● Overseeing day-to-day service delivery across our NDIS, Aged Care and Veteran (DVA/VHC) programs

● Managing rostering, performance and supporting our support workers and Delivery Partner network

● Leading care and service plan coordination, and acting as the first point of contact for clients and families on service delivery matters

● Managing NDIS Plan Management - processing and monitoring participant plan budgets, invoicing accuracy and reporting

● Owning compliance and quality at the coordination level - NDIS Practice Standards, Aged Care Quality Standards, incident management and documentation

● Partnering with the Operations Manager on service improvements,



systems and the initiatives that support our growth - including expanding our Delivery Partner network as we take on new regions

About You

● Experience in care coordination, case management or client/service management within NDIS, Aged Care or veteran/DVA-funded programs

● Working knowledge of NDIS Practice Standards and/or Aged Care Quality Standards

● Experience with NDIS Plan Management or financial administration of client budgets is highly regarded

● Proven ability to lead, roster and support a team of support workers or care staff

● Solid relationship-building skills with clients, families, Delivery Partners and referrers

● A relevant qualification in community services, health, social work, business or a related field (or equivalent experience)

● Current driver's licence, NDIS Worker Screening Check / Working with Vulnerable People Check and National Police Check (or willingness to obtain)

● $85,000 – $95,000 + Superannuation, dependent on experience
