We are seeking a motivated and compassionate
Enrolled Endorsed Nurse (EEN)
to join our Community Nursing team. In this role, you will provide comprehensive nursing care to
Home Care Package (HCP) recipients
in their own homes. You will work under the guidance of a Registered Nurse, delivering clinical services and supporting clients to manage their health and wellbeing safely at home.
Key responsibilities
Deliver in-home nursing care including wound care, medication administration, continence care, and chronic disease management.
Complete initial and ongoing
clinical assessments
and
regular care reviews
to ensure care plans remain responsive to clients' changing needs.
Conduct




risk assessments
to identify and mitigate potential hazards in the home setting.
Perform
post-hospital discharge visits
and
post-falls assessments
to support recovery and reduce the risk of readmission.
Monitor and report changes in clients' health status to the supervising Registered Nurse.
Educate and support clients and their families in managing health conditions and following care plans.
Maintain accurate and timely clinical documentation and care notes.
Work collaboratively with the broader Home Care team to provide coordinated and holistic care.
Ensure compliance with all clinical standards, safety regulations, and aged care guidelines.
