Life Without Barriers is seeking a Child and Family Practitioner in Hobart, TAS for the Placement Preservation Support Program. You will develop Family Action Plans for families under statutory orders, using evidence-based interventions and an assertive outreach approach to promote placement stability.
You will work with Parents/Caregivers, children, and relevant agencies to support positive long-term outcomes, coordinating care team meetings and documenting progress.
