Kyabram District Health Service is seeking a Care Coordinator for the Seniors Lifestyle Victoria Program. You will assess frail older adults, identify goals and develop action plans using a person-centred approach, coordinating services with GPs, allied health and community partners.

Qualified Allied Health (Grade 3) or Nursing skilled with strong clinical skills is preferred. You will document progress, support access and transitions, and contribute to program planning in a rural health
