APM is seeking a Clinical Aged Care Assessor to identify needs of older adults and connect them with services to help them live independently, working 4 days per week. Daily travel across the service area is required.
You will conduct high-quality assessments, facilitate referrals, and promote continued function while engaging with families and carers, within Department of Health contracts. This role offers multidisciplinary collaboration and dedicated training.
