Job responsibilities:

- Support protected transitions of care, including handoffs and care plan updates.
- Work 36 hours per week with 13-hour shifts.
- Coordinate patient care activities with physicians and other clinical team members.
- Prepare patients for diagnostic tests or procedures and provide post-procedure monitoring as indicated.
