Western Health is seeking an experienced Emergency Department Care Coordinator to join the EDCC Team on a 12-month fixed-term, full-time basis. The role focuses on improving patient outcomes by reducing avoidable hospital admissions and coordinating care for patients with complex needs in the Emergency Department.
You will work in a multidisciplinary team, develop individualised management plans, and support secure discharges with trauma-informed, person-centred care. Closing date: 2 August 2026.
