Uniting NSW.ACT is seeking a Support Coordinator to assist participants with NDIS plans, navigate portals and establish service agreements across multiple sites. This 9‑month, full time role involves 38 hours weekly, with the option to work from home twice weekly and travel to North Parramatta, Leichhardt or Campbelltown as required.

You will apply person-centred approaches, liaise with providers and Plan Managers, and help participants access mainstream supports, while maintaining strong
