About the role

We are seeking an enthusiastic, positive and team orientated leader to join our team on a full-time basis at our Mooroolbark office.

This is a rewarding and diverse role responsible for coordinating the client journey from initial enquiry through assessment, service commencement and ongoing review. Working closely with people living with dementia, their families, My Aged Care, health professionals and our internal team,



you will ensure services remain responsive, person-centred and aligned with current aged care requirements.

You will play a key role in coordinating client intake and assessments, supporting carers, developing person-centred Support Plans, building strong relationships with referral partners and contributing to the ongoing development of our dementia support programs.

The Program Coordinator also contributes to program development, continuous improvement and service quality, helping Caladenia continue to deliver exceptional dementia support services.

Key responsibilities

- Manage enquiries from prospective clients, carers and referral agencies.

- Undertake comprehensive dementia-informed client assessments.

- Develop person-centred Support Plans and Behaviour Support Plans where required.

- Coordinate referrals and support clients throughout their service journey.

- Conduct ongoing client reviews and respond to changing support needs.

- Liaise with carers to ensure high-quality and responsive service delivery.

- Coordinate with My Aged Care, General Practitioners, Memory Clinics, Allied Health professionals, hospitals and community organisations.

- Coordinate and support the delivery of Caladenia's dementia and carer support programs.

- Assist with waiting list management and client service allocation.

- Maintain accurate electronic client records and documentation.

- Participate in multidisciplinary case reviews and continuous improvement initiatives.

- Contribute to the development of education resources, community information sessions and new service initiatives.

- Ensure services are delivered in accordance with the Aged Care Quality Standards,



Support at Home requirements and organisational policies.

What we're looking for

- Qualification in Nursing, Social Work, Occupational Therapy, Psychology, Community Services or a related discipline.

- Experience working with people living with dementia or older people.

- Experience undertaking client assessments and case coordination.

- Excellent communication, relationship-building and interviewing skills.

- Demonstrated ability to build strong relationships with clients, carers and external stakeholders.

- Strong organisational skills with the ability to manage competing priorities.

- High-level written documentation and report writing skills.

- Ability to work both independently and collaboratively within a multidisciplinary team.

- Commitment to person-centred practice and continuous improvement.

- Proficiency with Microsoft 365 and electronic client management systems.

Preferred experience

- Experience working within dementia-specific services.

- Knowledge of My Aged Care and the Support at Home Program.

- Experience developing Behaviour Support Plans.

- Understanding of Younger Onset Dementia.

- Experience using client management systems.

- Knowledge of the Aged Care Quality Standards.
